Check Requests - Disbursements
If you need to submit a check request, please send all .pdf requests to shelby@laneccfoundation.org and cc: jeri@laneccfoundation.org. Please attach appropriate back-up. If the payment is for an individual, a completed W-9 needs to be attached as well. More information regarding disbursements.
