Note: If your application has been selected for verification of information, you must access the required forms through your myLane account under the myFinances tab.

Students that are required to file an appeal or wish to request changes to their financial aid may select the appropriate forms from the links below.

Appeals

If you have received notification that an appeal is required, use the appropriate fillable form below. To determine if you are required to submit an appeal, check your myLane account under the myFinances tab; student requirements; and then the HOLD tab. Complete all forms in full, attach required documentation and sign. Incomplete, unsigned, or non-required forms will not be processed and will delay the processing of your aid.

Appeal Forms

Maximum Timeframe Appeal – Complete the Maximum Timeframe portion of this form if you have been notified you may exceed the maximum credit limit.

Developmental Credit Limit – Complete this form if you have a Developmental Credit Limit hold on your Financial Aid account.

Satisfactory Academic Progress Appeal - If your financial aid status is "suspended", complete the Satisfactory Academic Progress portion of this form to explain circumstances that prevented you from meeting Satisfactory Academic Progress Standards.

Requests

The following forms are used to request changes or updates to your financial aid. Please read form descriptions to determine if you are using the correct form and if they apply to your circumstances. Be sure to read all form instructions and select the correct academic year form before submitting. If you are unsure you are submitting the appropriate form, please contact Financial Aid for assistance.

Request Forms
2020/2021 Child Care/Dependent Care Budget Request - This form is required if you are requesting that your cost of attendance be increased due to child (under age 13) or dependent care expenses. 
Financial Aid Consortium Agreement - This form is used for students who are concurrently enrolled at Lane CC and another college/university and would like credits combined for financial aid eligibility.
Oregon Grant Waiver Request - Use this form to request a waiver to one of the requirements for the Oregon Grants.
Pell Adjustment Request - Use this form to request a Pell Grant adjustment if an institutional error prevented enrollment in the correct number of credits at the time a Pell Grant was calculated for the term.
Preparatory Loan Funding Request - Students completing prerequisites to apply for admission to limited enrollment programs may use this form to be considered for preparatory loan funding. An information sheet is attached to the request to help you determine if this is the right option for you.
Refusal to Provide Information and Support - Parent(s) may complete this form if they are refusing to provide required FAFSA information.
Request For Change – Use this form for miscellaneous requests to Financial Aid, such as enrollment changes, cost of attendance changes, or accepting previously declined loans.
Request To Apply For Financial Aid Without Parent Information – Dependent students may use this form if requesting to be considered an Independent student. See Dependent vs. Independent to determine if you may be eligible.
Request to Increase Budget for Computer - Complete this form if you would like to increase your student budget for the purchase of a computer.
2019-2020 Special Circumstances Request, Student Income - Use this form if there has been a significant change in financial information since you filed the FAFSA.
2019-2020 Special Circumstances Request, Parent(s) Income - Use this form if there has been a significant change in parent(s) financial information since you filed the FAFSA.
2020-2021 Special Circumstances Request, Student Income - Use this form if there has been a significant change in financial information since you filed the FAFSA. 
2020-2021 Special Circumstances Request, Parent Income - Use this form if there has been a significant change in parent(s) financial information since you filed the FAFSA.
2020-2021 Unsubsidized Stafford Loan Request - Complete this form to request an offer of the Federal Direct Unsubsidized Stafford Loan. Submit ONLY if you have already been offered for 2020-2021.

19/20 Pell Grant Payment Chart - Pell Grant students may use this chart to estimate their Pell Grant offer if attending less than full time for the 2019-2020 academic year. 
20/21 Pell Grant Payment Chart - Pell Grant student may use this chart to estimate their Pell Grant offer if attending less than full time for the 2020-2021 academic year.
Repeat Coursework - This flow chart can be used as a guide to help determine if a repeated course can be funded.
Maximum Subsidized Loan Eligibility - There is a limit on the maximum period of time a new student borrower can receive the Direct Subsidized Loan. See the Federal Student Aid website for details.
